Audience Feedback Questionnaire after Seeing the Opening Sequence

I wanted to gauge audience response after finishing editing my opening sequence, to see if they felt that it reflected the genre, and to find out if they had been hooked enough to want to carry on watching the film, to see what happened in the narrative.
